Registration is done at the Watonga Elementary front office. To register you will need the following information:

Current Shot Record               Proof of Residency                Birth Certificate        Social Security Card

5 DTp's, 4 Polio's, 

2MMR's, 1 Varicella

2 HEP A's, 3 HEP B's
